Student Annual Forms – Deadline September 18:

The Student Annual Forms are now available in your MyCBE Account.   Please sign in to your MyCBE/PowerSchool account, select Online Forms, select Student  and complete the following:

  • Demographic Verification 2023-2024
  • Annual Authorization 2023-2024
  • Citizenship Information Update 2023-2024 (if applicable)

We are required by the Alberta Education to have these forms completed.  

 

Lunch Supervision Registration – Deadline Sept. 30:

All students remaining at school during the lunch hour MUST be registered for Lunch Supervision via your MyCBE account, Fees, Lunch Supervision.

 

Updating Emergency Contacts:

We are unable to update emergency contact information over the telephone or via email.  Please update the 2023-2024 Student Demographic form which is in your MyCBE Account.  If your form has already been approved by the school, we can send you a new one to update.

 

New Volunteer Process & Costs:

If you wish to become a volunteer, you will now be responsible for the cost of ePIC registration.  The cost is $21.25 per person. 

On a receipt of a completed annual volunteer form, and two forms of government ID (one being a picture ID) the school will send you a letter and a link to apply for your ePIC.   You have 180 days to complete the ePIC process.


Once your ePIC has been processed by the CPS they will send you a letter confirming the outcome of the registration. 

You MUST bring a copy of this letter to the school or email Sue Naylor senaylor@cbe.ab.ca a copy of this letter.    On receipt we will update our records and you will be ready to volunteer.

Volunteers who have lived outside of Calgary MUST obtain a PIC including Vulnerable Sector Check from RCMP or police agency in the jurisdiction where you lived, this must be submitted to us at the school.


New requests will be processed in October.

 

Transportation, Bus Routes & MyBusStop App:

Bus stops and routes will be posted in your MyCBE account when your registration has been completed.

If you have not yet registered for transportation, please sign into your MyCBE account, Fees Transportation Registration.  Your registration may take up to 6 weeks to process.

If your child is riding the bus, we recommend you register for MyBusStop app. The app will notify you of delays and track your child's bus. The school code you require is: DDXT

As part of back-to-school transportation planning, we encourage all families to have back-up plans in place in case of delayed or cancelled bus service during the school year. Families are responsible for the safe passage of students to and from the bus stop and at the bus stop.

 

Healthy Lunch & Snack:

Please send your child(ren) with a healthy lunch, snack and water bottle each day.  The office is very busy during lunchtimes therefore, we ask you to avoid dropping off lunches at this time.

Just a reminder that Douglasdale School is a Nut Aware school.

 

Student Medication:

If you require medication to be stored at school, please complete the attached form along with the medication in a ziplock bag containing the child’s name.

Serve Medical Conditions:

If your child has been diagnosed with a serve medical condition, please complete the attached form and return to the office ASAP.

Medical forms must be revised annually.

 

Reporting Absences / Lates:

If your child is going to be late or absent, please either call the school office 403-777-6177 Ext. 1 or email Ms. Naylor senaylor@cbe.ab.ca before 9:05am.  Please speak clearly and slowly when leaving a message and include:

1. Your child’s name

2. Teacher's name 

3. Reason for the absence or late

Please email your child's teacher and Ms. Naylor senaylor@cbe.ab.ca  if you are picking your child up early from school.
